Review of Pocket BVM by Doc Delta

Aquí minireview comparativa del dispositivo MicroBVM que ha despertado cierto hype por su utilización en entorno tactico por llevarlo unidades médicas de OperacionesEspeciales .Esta review se plantea desde un punto de vista profesional y NO COMERCIAL valorando los pros y contras de dicho dispositivo aportando datos cualitativos y cuantitativos comparados con otro “ ambu ” o balón de resucitación de propiedades parecidas como es el BVM para adulto en tamaño pequeño. Empezamos!

1. Tamaño:es la principal característica ya que ocupa un espacio muy pequeño comprimido ocupando solo 8 centimetros en su envase lo que lo hace ideal para llevar en una mochila.Si comprimimos el otro dispositivo obtenemos unos 20 centímetros con suerte.

2. Envase:nos ofrece la ventaja de estar protegido ante impactos o el roce de otro material además de ser mas higíenico frente al resto de dispositivos que solemos llevar solos o en su bolsa y con la actual situación frente Covid lo veo adeacuado.

3. Peso:que sea mas pequeño no quiere decir que pese menos ya que pesa 450 gramos aprox. frente al dispositivo utilizado de comparación que pesa unos 420 gramos.

4 Materiales:la calidad de los materiales es muy buena,me sorprendío gratamente,me costó desplegarlo la primera vez ya que lo dejé en el coche al frío y la goma estaba dura pero se ve un dispositivo robusto aunque el reservorio no me ofrece mucha confianza de resistencia ya que es una bolsa de plástico delgado que puede rasgarse con facilidad si no tienes cuidado y no se puede tirar para desplegar ya que se rompería y tampoco se puede cambiar para poner otro estándar con en la mayoría de estos dispositivos.

5. Características:el tamaño de la boquilla es estándar como cualquier dispositivo de ventilación,la mascarilla es de silicona de buena calidad aunque prefiero las mascarillas hinchables ya que se suelen amoldar mejor a la forma de la cara de las personas pero es una opinión personal,la ventaja es que está en el envase y no se suele deformar como las que solemos llevar en la mochila al estar mucho tiempo con el resto de materiales.Ofrece poder colocarle valvula PEEP y filtro pero carece de valvula de sobrepresión que nos indica que hay una obstrucción en la vía aérea o en el tubo IOT.Incluye toma para conectar oxígeno pero dependiendo de la versión incluye tubo auxiliar o no.No ofrece asa para fácil sujeción pero al menos la superficie ofrece superficie antideslizante.

6. Montaje:es muy sencillo y rápido,abrir,desplegar por la base sin tirar del reservorio y colocar mascarilla.Ofrece algo de resistencia por los materiales pero no es un problema.A la hora de volver a plegarlo es más complicado y requiere tiempo aunque estamos hablando de un dispositivo de “un solo uso” es decir que es usar y tirar…

7. Precio:aquí veo el principal inconveniente ya que su precio ronda los 40€ frente a dispositivos que cumplen con su objetivo de un solo uso que rondan los 10€.

8. Complementos:me ha resultado muy útil el tubo auxiliar extensible que permite alargar hasta 75 centímetros la distancia entre el dispositivo y la mascarilla que favorece mucho la RCP instrumentalizada ante COVID que recomienda un operador manteniendo mascarilla pegada a la cara del paciente para evitar fugas de aire y aerosoles potencialmente contaminantes y otro operador realizando compresiones y ventilado con el dispositivo que puede dejar cerca y no perder tiempo,además de aliviar el peso del dispositivo sobre la mascarilla y facilitar la maniobra de sellado al operador de cabeza.

Conclusión y opinión personal/profesional:se trata de un dispositivo innovador cuya principal ventaja es la protección e higiene que nos ofrece el envase rígido pero que al fin y al cabo cualquier ambú o bolsa de reanimación realiza las mismas funciones a un coste/calidad más ajustados.Lo veo útil para llevar en la mochila de ataque o apoyo respiratorio porque va protegido y ocupa poco volumen,peso es prácticamente el mismo que cualquier dispositivo y otra ventaja es que no se va a deformar por estar en la mochila con el resto de material por lo que su aplicación será óptima a la hora de utilizarlo y que en su envase está prácticamente hermético y puede limpiarse y desinfectarse sin problemas si se encuentra en el exterior(ozono o peróxido de hidrógeno)a la hora de traslado de pacientes COVID+. Es un dispositivo de un solo uso por lo que no se recomienda su reutilización(aunque todos sabemos lo que sucede en realidad en las dotaciones y que las bolsas de resucitación se limpian, desinfectan y reutilizan).Me ha resultado muy útil el tubo corrugado extensor de 75 centímetros que lo veo un acierto frente RCP en paciente con sospecha de COVID o COVID+.

Preguntas y sugerencias:si teneis alguna pregunta podeis comentar los post de esta minireview del dispositivo MICROBVM y las iré resolviendo según hagáis las preguntas ya que mi valoración está enfocada al SVB y dejando a un lado la tendencia actual de utilizar material tacticalcool en el entorno sanitario frente a material sanitario de toda la vida que ofrece mismas características sin que por ello dejes de ser menos profesional. Parafraseando al compañero medicotactico tenemos que dejar de ser tan tacticalcool y ser más medicool.

5 Characteristics: the size of the mouthpiece is standard like any ventilation device, the mask is made of good quality silicone although I prefer inflatable masks since they tend to mold better to the shape of people’s faces. But it is a personal opinion, the advantage is that it is in the container and does not tend to deform like the ones we usually carry in our backpack when we spend a lot of time with the rest of the materials. It offers to be able to place a PEEP valve and filter but it lacks an overpressure valve It indicates that there is an obstruction in the airway or in the IOT tube. It includes a socket to connect oxygen but depending on the version it includes an auxiliary tube or not. It does not offer a handle for easy grip but at least the surface offers a non-slip surface.

It is a single-use device so reuse is not recommended (although we all know what actually happens in endowments and that resuscitation bags are cleaned, disinfected and reused). I have found the corrugated tube extender very useful of 75 centimeters that I see a success against RCP in a patient with suspected COVID or COVID +.
